Nomination Information

ALL FIELDS ARE REQUIRED. Incomplete information may disqualify your nomination.

This form must be completed by the Director/Coordinator of the Nominee’s program.

The nomination must include a statement of support from an adult student who works directly with the Nominee.

Nominations may be submitted online or sent via email to lavetisian@famlit.org.

Programs may nominate up to two (2) educators, but must submit separate online nomination forms. The Nominated Educator cannot have been a finalist in the past three years (2007, 2008, or 2009).

The nomination deadline is February 24, 2010, at midnight Eastern Standard Time.
